Hillside Wellness Center emphasizes a collaborative, client-centered approach that fosters growth, stability, and sustained wellness for each person served.Role Description The Admissions and Case Management Coordinator is a full-time, on-site role based in Fallsington, PA. This position is responsible for coordinating the intake and admissions process, including responding to inquiries, screening prospective clients, gathering documentation, and ensuring a smooth transition into the program. The coordinator will manage client records, develop and maintain case files, track treatment progress, and support discharge planning in collaboration with clinical staff. Daily tasks include communicating with clients, families, and referral sources; scheduling assessments and appointments; and maintaining accurate, timely documentation in accordance with regulatory and organizational standards. The role also involves supporting quality client experience, assisting with insurance or payment-related information, and working closely with the multidisciplinary team to promote continuity of care.QualificationsExperience in admissions coordination, case management, or client services within behavioral health, social services, or a related field.Strong organizational, documentation, and time management skills, including the ability to manage multiple cases and deadlines.Excellent written and verbal communication skills, with a focus on compassionate, professional interaction with clients, families, and referral partners.Proficiency with electronic health records (EHR), scheduling systems, and basic office software (e.g., Word, Excel, email).Knowledge of mental health and substance use disorder treatment settings, along with familiarity with confidentiality and compliance standards (e.g., HIPAA).Ability to work collaboratively in a multidisciplinary team and to adapt to changing client needs and program priorities.High level of integrity, discretion, and commitment to person-centered, non-discriminatory care.Associate or bachelor’s degree in social work, psychology, counseling, healthcare administration, or a related field preferred; equivalent experience considered.Prior experience working in a residential or inpatient setting and with insurance verification or utilization review is a plus.
