The campus spans 56 acres and offers a range of living options, including Sterling Park Independent Living apartments and garden homes, Assisted Living apartments, the H.O.P.E. Center for Memory Care, and private rooms in the Pavilion for short-term rehabilitation or long-term skilled nursing care. Residents also have access to outpatient rehabilitation services on-site. Osborn Home Care, a New York State licensed home care agency, provides registered nurse case management and personal care services by certified home health aides in Westchester County and on The Osborn campus.Role Description The Assisted Living Care Coordinator is a full-time, on-site role based in Rye, NY. This role is responsible for coordinating care and services for assisted living residents, including developing and updating care plans, monitoring residents’ needs, and collaborating with nursing staff, home care professionals, and other interdisciplinary team members. The Care Coordinator serves as a primary point of contact for residents and their families, ensuring clear communication, responsive service, and a high standard of resident satisfaction. Daily tasks include conducting assessments, documenting care activities, arranging external services when needed, supporting transitions between levels of care, and promoting a safe, supportive living environment.Qualifications Strong Care Coordination and Case Management skills to develop, implement, and monitor individualized care plans.Excellent Communication and Customer Service abilities to interact effectively with residents, families, and interdisciplinary team members.Experience with Home Care services, including understanding of home health regulations, community resources, and collaboration with external providers.Knowledge of assisted living, memory care, or skilled nursing environments and related best practices.Ability to perform thorough assessments, maintain accurate documentation, and use electronic health record or care management systems.Organizational and problem-solving skills to manage multiple residents’ needs and prioritize tasks in a fast-paced setting.Relevant licensure or certification (such as RN, LPN, social work, or care management credentials) preferred.Prior experience in senior living, geriatric care, or a similar healthcare setting is highly beneficial.
