Our platform generates rich, high-frequency clinical data at the bedside, and for that data to change how care is delivered it must flow reliably into the systems clinicians already use. We are expanding our team to include a Backend Engineer who can design and build the integration layer that connects our platform to hospital EHRs, laboratory and device systems, and national health data exchanges.This role is ideal for a hands-on backend engineer with strong experience building production APIs and services, deep familiarity with healthcare interoperability standards such as HL7 FHIR and HL7 v2, and a track record of shipping and supporting integrations in live hospital environments. Experience with EHR vendor integration (e.g., Epic, Oracle Health/Cerner, InterSystems) and regional health information exchange requirements is highly preferred.This position is remote but requires travel across the Middle East to collaborate with the FluidAI team, hospital IT departments, and clients.Responsibilities🔌 Design, build, and maintain the integration services that exchange clinical data between the FluidAI platform and hospital systems (EHRs, LIS, ADT feeds, device gateways, and national health exchanges).🧬 Implement and maintain HL7 FHIR APIs and resources (e.g., Patient, Encounter, Observation, Device, DiagnosticReport) as well as HL7 v2 message interfaces (ADT, ORU, ORM) for inbound and outbound data flows.🏗️ Build clean, well-tested, production-grade backend services and APIs (REST and event-driven) that are secure, observable, and resilient to the realities of hospital networks.🗺️ Own data mapping and transformation logic between FluidAI's internal data models and external standards, including terminology mapping (LOINC, SNOMED CT, UCUM) and vendor-specific profile variations.🔁 Develop robust interface engine and integration patterns: message queuing, retries, deduplication, acknowledgement handling, idempotency, and reconciliation.🏥 Lead the technical side of hospital onboarding: gather interface requirements, conduct connectivity and message testing with hospital IT and EHR analysts, and support go-live and hypercare.🔍 Implement monitoring, alerting, and logging for integration health so that message failures and data gaps are caught early and traced end-to-end.🔐 Apply healthcare security and privacy best practices, including OAuth2/SMART on FHIR, mutual TLS, VPN connectivity, audit logging, and data minimization, in line with regional regulations (e.g., Saudi NPHIES/SeHE requirements, HIPAA, PIPEDA).📚 Create and maintain clear technical documentation: interface specifications, FHIR implementation guides, integration runbooks, and data flow diagrams.🤝 Partner with product, clinical, regulatory, and QA teams to ensure integrations meet clinical workflow needs and medical device software standards (e.g., IEC 62304, ISO 13485 processes).🏃♂️ Participate in sprint planning and Agile ceremonies, contributing to roadmap planning and prioritization from an integrations and platform perspective.🔧 Collaborate closely with software, hardware, data, and ML teams to expose reliable data services across the full technology stack.Qualifications📚 Strong hands-on experience as a Backend Engineer, Integration Engineer, or Interoperability Engineer, ideally within healthcare or medical technology.🧬 Deep working knowledge of HL7 FHIR (R4 and/or R5), including resource modelling, profiles, extensions, search, and bulk data, and hands-on experience with FHIR servers or SDKs (e.g., HAPI FHIR, Firely, Microsoft/Azure FHIR, Google Cloud Healthcare API).📨 Solid experience with HL7 v2.x messaging, including message structure, segments, ACK/NACK handling, and common workflows (ADT, ORU, ORM, MDM).🔌 Experience with integration/interface engines such as Mirth Connect (NextGen Connect), Rhapsody, InterSystems, Cloverleaf, or equivalent custom pipelines.💻 High proficiency in at least one backend language such as Python, Java/Kotlin, C#/.NET, Go, or TypeScript/Node.js, with strong API design skills (REST, OpenAPI, webhooks) and familiarity with message brokers (e.g., Kafka, RabbitMQ, MQTT).🗄️ Strong SQL and data modelling skills with relational databases (PostgreSQL, SQL Server, Oracle) and familiarity with NoSQL or time-series stores.☁️ Experience deploying and operating services in cloud and hybrid environments (AWS, Azure, or GCP; Docker; Kubernetes; CI/CD; infrastructure as code), including on-premise hospital deployments.🔐 Working knowledge of healthcare security and identity standards: OAuth2/OIDC, SMART on FHIR, TLS/mTLS, encryption at rest and in transit, and audit requirements.🏥 Direct experience integrating with EHR vendors (Epic, Oracle Health/Cerner, MEDITECH, InterSystems TrakCare, or regional systems) and working alongside hospital IT teams is highly preferred.🧠 Familiarity with clinical terminologies (LOINC, SNOMED CT, ICD-10, UCUM) and with medical device connectivity standards (e.g., IEEE 11073, IHE profiles such as PIX/PDQ, XDS) is a strong plus.📋 Familiarity with regulated software development practices (IEC 62304, ISO 13485, risk management, traceability) is a plus.🗣️ Fluent in English; Arabic is a strong plus.
