Our AI platform and care model support patients between visits, helping them understand their risks, take the next right step, and get healthier over time.We are seeking a Behavioral Health Coach to deliver CBT-informed lifestyle and behavior-change coaching for patients managing chronic health risk factors. This is not psychotherapy and not traditional care management. Our platform handles clinical registry, documentation, and care-management workflows. Your role is to deliver human, high-impact micro-interventions that help patients build sustainable habits across diet, exercise, sleep, stress, and emotional health.The ideal candidate will have strong interpersonal and communication skills, be comfortable conducting video and phone sessions, and have experience motivating patients toward sustainable behavior change.Note: We are currently focusing and prioritizing bilingual applicants who are fluent in Turkish. Please only apply if you can speak Russian fluently - all other applicants will not be considered.Key ResponsibilitiesManage a virtual caseload of patients, supporting them in modifying risk factors that influence chronic conditionsCollaborate remotely with primary care physicians and psychiatric consultants to develop and update personalized care plansDeliver brief, structured evidence-based behavioral interventions, including motivational interviewing, behavioral activation, and goal setting in 15-30 minute sessions.Review patient-reported data and adjust coaching strategies as neededEngage patients through secure messaging and weekly video/phone sessions to sustain progress and habit formationEducate patients on health risk factors and techniques for sustainable lifestyle improvementSupport adherence to medication and treatment plans to optimize health outcomesAccurately document all encounters and maintain compliance with HIPAA and program standards
Requirements
Must be based in the United States and legally eligible to work in the U.S.Experience in behavioral health coaching, care management, or related health fields (e.g., psychology, social work, counseling, nursing)Excellent virtual communication and engagement skillsProven ability to motivate patients and facilitate behavioral changeFamiliarity with telehealth platforms, digital engagement tools, and electronic health recordsUnderstanding of habit formation frameworks (e.g., “Tiny Habits,” CBT-based methods, motivational interviewing, behavior activation)Preferred knowledge of Collaborative Care Model (CoCM) and integrated behavioral health servicesStrong empathy, cultural sensitivity, and ability to support diverse populationsSelf-motivated with high accountability in a remote work environmentBackgrounds we commonly hire from:Health coaches trained in CBT/MITherapists who prefer coaching over psychotherapyNurses or allied health professionals with behavior-change experienceAbout Mederva HealthMederva is a health technology company that integrates lifestyle medicine and behavioral therapy into medical practices, supporting patients with personalized guidance between visits to help them live longer and healthier lives.
