As a Bilingual Referrals Coordinator, you will be at the heart of our mission, helping patients navigate their healthcare journey with warmth and compassion.Role Highlights: Be the vital link between our clinical staff and external providers, ensuring seamless referrals for our patientsWork Environment: Thrive in a supportive and collaborative team setting where your contributions truly matterImpact: Play a crucial role in overcoming financial and social barriers to care, ensuring every patient receives the support they needGrowth Opportunities: Engage in continuous learning and personal development within our dynamic healthcare ecosystemMission-Driven: Join us in our commitment to providing high-quality, affordable healthcare to the communities we serveKey ResponsibilitiesReview and process all diagnostic, specialty, and miscellaneous referral requests from clinical staffObtain patient insurance verification and authorization for referrals, scheduling appointments as neededConduct phone consultations with clients regarding their referral statusHandle prior authorization requests from clinical staff efficientlyKeep accurate records using Centricity EHRSBuild and maintain strong relationships with providers and referral sourcesAdhere to regulations for a Patient Centered Medical Home and HIPAA GuidelinesMaintain knowledge of and comply with Medicare/Medicaid regulationsEnsure proper documentation of service coordinationFollow up and complete orders for referred patientsInitiate and complete forms for provider review and signatureProvide phone contact and motivation for patients needing imaging services and preventative cancer screeningsSend reminder phone calls and letters to patientsSet and confirm medical appointments as neededParticipate in our Performance Improvement Program for care coordinationPerform other duties as assigned, contributing to the overall success of the teamMinimum QualificationsHigh school diploma or GEDOne year certificate from college or technical school; or three to six months related experience and/or training; or equivalent combination of education and experienceOne year of experience in medical assistance, referral coordinator, insurance verification, or medical prior authorizationAbility to communicate effectively, both written and orallyExcellent customer service skillsAbility to multi task in a fast paced environmentExcellent organizational skillsStrong typing and computer skillsMust be computer savvy with strong MS Office knowledge, including Word, Excel, Outlook, and Power PointProfessional written and verbal communications skillsSpanish speaking requiredAbout Legacy Community HealthAs the largest Federally Qualified Health Center (FQHC) in Southeast Texas, Legacy Community Health has been delivering comprehensive, high-quality, and affordable health care for nearly 40 years.
