This role serves as an advocate, educator, and care coordinator, helping members navigate healthcare resources, overcome barriers to care, and actively participate in their healthcare journey.The ideal candidate has experience working with diverse populations, strong communication skills, and a passion for improving health outcomes through coaching, care coordination, and member engagement.ResponsibilitiesCare Coordination & Member SupportAssess member needs and develop individualized care plansGuide members and families toward healthcare, community, and social service resourcesIdentify and address barriers that may impact health outcomes, treatment adherence, or access to careSupport members in achieving healthcare goals through education, coaching, and ongoing engagementMonitor member progress through assessments, data review, and direct communicationCollaborate with healthcare providers, caregivers, and community partners to coordinate servicesHealth Coaching & WellnessUtilize coaching techniques to promote behavior change and healthy lifestyle choicesSupport chronic condition management and preventive health initiativesEncourage member participation in wellness and health promotion programsAssist members in understanding available healthcare benefits and resourcesDocumentation & Case ManagementDocument member interactions, care plans, and progress accurately in electronic systemsMaintain case notes and records across multiple platformsTrack outcomes and support continuous improvement of care management activitiesFollow established policies, procedures, and professional standardsMember AdvocacyServe as a resource and advocate for members and familiesPromote positive healthcare experiences and improved care outcomesCommunicate effectively with members, providers, and internal stakeholdersSupport initiatives focused on consumer satisfaction and quality improvementRequired QualificationsBachelor's degreeMinimum of 2 years of related experience in healthcare, care coordination, case management, social services, wellness coaching, or a related fieldStrong knowledge of Microsoft Word and ExcelExcellent verbal and written communication skillsStrong interpersonal and relationship-building abilitiesAbility to learn and utilize healthcare technology and electronic systemsExperience working with multiple software applications and databasesValid state driver's licenseReliable personal vehicleProof of personal automobile liability insurance with minimum coverage of:$100,000 bodily injury per person$300,000 bodily injury per accident$100,000 property damage liabilityAbility to complete required Tuberculosis (TB) screeningPassion for improving consumer experiences and health outcomesPreferred QualificationsMaster's degreeExperience working with Medicare and Medicaid populationsExperience documenting electronic case notes and working across multiple healthcare systemsExperience with health coaching, wellness programs, or care managementKnowledge of community health agencies, social service organizations, and local community resourcesExperience supporting vulnerable or complex-care populationsKey SkillsCare CoordinationCase ManagementHealth CoachingMember AdvocacyCommunity Resource NavigationWellness PromotionCare PlanningPatient EngagementDocumentation & Case NotesCustomer ServiceRelationship ManagementProblem SolvingCommunication
