You will work closely with registered nurses and a wide range of healthcare and community professionals to coordinate referrals, support care pathways and help patients remain in their preferred place of care wherever possible.You will also play an important role in supporting joined-up working and helping to prevent avoidable hospital admissions through effective communication and coordination between services.About the roleAs a Care Co-ordinator, you will:Coordinate referrals and support patient care pathways.Triage and prioritise referrals alongside a registered nurse, considering urgency, complexity and risk.Act as a key point of contact for patients, families, carers and healthcare professionals.Respond to clinical enquiries by telephone and provide appropriate information and signposting.Ensure patients receive a welcome call and relevant service information.Monitor and manage referrals and email communications.Support the smooth flow of information between healthcare services and professionals.Liaise with GPs, community teams, ambulance services, hospices, clinical specialists, social care and voluntary sector organisations.Recognise signs of deterioration, red flags and safeguarding concerns, escalating appropriately.Promote coordinated, person-centred care for patients and their families.Maintain accurate records and report accidents and incidents appropriately.Work within your scope of practice while maintaining confidentiality and professional standards.Contribute to the delivery of a high-quality palliative and end-of-life care service.About youYou will have experience of coordinating appointments, referrals or patient pathways, ideally within a healthcare or community setting.
