About the RoleAs a Care Manager, you will be the primary point of contact for a dedicated panel of patients enrolled in TOCA Health’s chronic care programs.You will build ongoing relationships with patients through regular outreach, remote vital-sign monitoring, care plan management, education, follow-up, and documentation. This role is ideal for someone who is compassionate, organized, comfortable speaking with patients on the phone, and confident working in a technology-driven environment.What You’ll DoConduct regular outreach calls to assigned patients and review their overall wellbeing and vital-sign trends.Review remote monitoring data, including blood pressure, glucose, weight, and oxygen readings.Identify patients who require follow-up or escalation to nurses, physicians, or other clinical staff.Develop and update individualized care plans for patients enrolled in Chronic Care Management.Educate patients on their care plans, medications, lifestyle goals, and chronic-condition management.Document patient interactions, care plan updates, and clinical communications accurately and on time.Use TOCA Health’s software and AI-assisted tools to support patient conversations, documentation, and workflow efficiency.Manage follow-up schedules, unresolved tasks, and monthly patient outreach requirements.Assist patients with basic device setup, connectivity, and remote-monitoring troubleshooting.Collaborate with physicians, nurses, care managers, and operational leadership as needed. What We’re Looking ForRequired1–2 years of experience in healthcare, a medical office, care coordination, or a similar patient-facing environment.Strong telephone communication skills and the ability to make patients feel comfortable and heard.Strong written communication and documentation skills.Excellent organization and the ability to manage a large patient panel.Comfortable learning new technology and software.Experience with Excel or Google Sheets.Reliable, self-directed, and able to take ownership of your responsibilities.Professional, energetic, and genuinely interested in helping patients.PreferredBilingual in English and Spanish.Certified Medical Assistant (CMA) or similar clinical background.Experience working in a physician practice or medical office.Familiarity with blood pressure, glucose, or other vital-sign monitoring.Experience with RPM or CCM.Experience using EMR/EHR systems.Familiarity with AI tools such as ChatGPT, Gemini, or Claude.
