Location: Sacramento, CAStart Date: September 2026Setting: Emergency Department & InpatientSchedule: Candidates will work 4 shifts per week, including:2 weekday shifts: either 8:00 AM–4:30 PM OR 2:00 PM–10:00 PM2 weekend shifts: 9:30 AM–6:00 PMWe are seeking an experienced Care Navigator to support patients and families within a fast-paced Emergency Department and Inpatient healthcare environment.This position is ideal for someone who is compassionate, confident, and comfortable independently engaging with patients and families while collaborating closely with nurses, social workers, and other members of the clinical care team.ResponsibilitiesEngage directly with patients and families in Emergency Department and Inpatient settingsIndependently introduce yourself to patients, identify needs, and provide appropriate supportConnect patients and families with available healthcare, social, and community resourcesCollaborate with nurses, social workers, and other clinical team members to coordinate servicesHelp patients navigate available programs, resources, and next steps in their careMaintain professional boundaries while providing compassionate, patient-centered supportDocument patient interactions accurately and thoroughlyEscalate clinical, social, or safety concerns to the appropriate member of the care teamRemain calm and professional when supporting patients and families in stressful situationsRequirements2–5 years of relevant professional experiencePrevious Emergency Department and/or Inpatient experience strongly preferredExperience working in a clinical healthcare environmentComfortable independently approaching and communicating with patients and familiesStrong verbal communication and interpersonal skillsAbility to effectively collaborate with nurses, social workers, and other healthcare professionalsStrong patient service and customer service skillsAbility to identify patient needs and connect individuals with appropriate resourcesStrong organization, prioritization, documentation, and follow-through skillsAbility to work independently while functioning effectively as part of a multidisciplinary teamProfessional, compassionate, and culturally sensitive approach to patient interactionsAbility to maintain patient confidentiality and follow healthcare policies and proceduresMust be available to work Saturdays and Sundays from 9:30 AM–6:00 PMEducationHigh school diploma or equivalent requiredPreferred QualificationsBachelor’s degree in Social Work, Psychology, Public Health, Human Services, Sociology, Gerontology, or a related fieldBilingual and/or bicultural skills reflective of the communities servedThis is a strong opportunity for someone with hands-on patient-facing healthcare experience who enjoys helping patients and families navigate resources while working as part of a collaborative clinical team.
