we are seeking a compassionate, detail-oriented Care Navigator (CNA, MA certification needed) to support our Chronic Care Management (CCM) program. In this role, you will serve as a key point of contact for patients with multiple chronic conditions, helping them navigate their care, stay engaged with their care plans, and prevent avoidable hospitalizations or ER visits. Our goal is to bridge the gap between clinics and patients. This position blends clinical knowledge, patient education, care coordination, and strong communication skills.

● Patient Outreach & Engagement ● Care Coordination & Navigation ● Care Plan Support ● Documentation & Compliance ● Medication & Health Monitoring Support ● Patient Advocacy & Support ● Patient-centered and empathetic approach to care ● Strong organizational and time-management skills ● Ability to work independently while collaborating with a multidisciplinary team ● Detail-oriented with a focus on accurate documentation ● Comfortable handling sensitive health information (HIPAA compliant) Required:● Certified Nurse Assistant or Registered Medical Assistant (CNA or MA or CHW) ● Minimum 1–2 years of clinical experience (primary care or care coordination preferred) ● Strong understanding of chronic disease states (e.g., diabetes, hypertension, CHF, COPD) ● Excellent communication and interpersonal skills, fluent in English and Spanish ● Proficiency with EHR systems and basic computer applications Preferred:● Experience in Chronic Care Management, care coordination, population health, or value-based care ● Prior remote or telephonic patient care experience ● Knowledge of CMS CCM requirements and documentation standards ● Bilingual (specify language if applicable)Reports To: Clinical Team Lead
