A leading group of hospitals is seeking a dedicated and experienced Case Manager to join our multidisciplinary healthcare team. The ideal candidate will have a strong nursing background, proven case management experience, excellent communication skills, and fluency in Arabic and English. Experience in Long-Term Care (LTC), rehabilitation, or chronic disease management is highly preferred.The successful candidate will play a key role in coordinating patient care, facilitating discharge planning, optimizing resource utilization, and ensuring continuity of care across the patient journey.Key ResponsibilitiesCoordinate and manage patient care plans throughout the continuum of care.Conduct comprehensive patient assessments to identify clinical, psychosocial, and discharge planning needs.Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure appropriate care delivery.Monitor patient progress and facilitate timely interventions to achieve optimal clinical outcomes.Manage discharge planning, referrals, and transitions to long-term care, rehabilitation facilities, home healthcare, or community services.Act as a liaison between patients, families, healthcare providers, and external agencies.Ensure effective utilization of healthcare resources while maintaining quality patient care.Maintain accurate case management documentation and reports.Support compliance with regulatory, accreditation, and hospital quality standards.Participate in multidisciplinary rounds and patient care conferences.RequirementsBachelor's Degree in Nursing from a recognized institution.Valid UAE Nursing License (DOH, DHA, or MOH) or eligibility.Minimum 3-5 years of clinical nursing experience, with at least 2 years of Case Management experience.Previous experience in Long-Term Care (LTC), Rehabilitation, Chronic Care, or Care Coordination is highly preferred.Fluent in Arabic and English (spoken and written).Strong knowledge of discharge planning, utilization management, and patient care coordination.Excellent interpersonal, communication, and problem-solving skills.Ability to work effectively within a multidisciplinary healthcare environment.Proficient in electronic medical records and healthcare information systems.
