Apply & track with Apply EdgeCase Manager – Intensive Mobile Treatment (IMT)Location: New York City, NYJob Type: Full-TimeWe are seeking a dedicated Case Manager to join our Intensive Mobile Treatment (IMT) program. This role provides person-centered, community-based support to individuals with complex needs who may be involved with homeless services, behavioral health, and criminal justice systems. IMT delivers flexible and continuous care wherever clients are located, including shelters, supportive housing, hospitals, correctional settings, or the community.Key ResponsibilitiesProvide case management, counseling, recovery planning, and crisis intervention.Develop individualized service plans and support clients in achieving personal and recovery goals.Assist with housing applications, housing placement, employment, public benefits, and community resources.Conduct outreach and accompany clients to appointments and services throughout New York City.Advocate for clients and coordinate services with healthcare providers, shelters, criminal justice agencies, and other community partners.Maintain accurate and timely case records, assessments, service plans, and required documentation.Manage an assigned caseload while prioritizing urgent client needs and maintaining consistent engagement.Participate in interdisciplinary team meetings and collaborate to ensure continuity of care and compliance with program requirements.QualificationsBachelor’s degree in Social Work, Psychology, Mental Health Counseling, Criminal Justice, or a related human services field required.Experience working with individuals experiencing mental illness, substance use, trauma, homelessness, or criminal justice involvement preferred.Strong assessment, communication, organization, problem-solving, and decision-making skills.Ability to work independently and effectively as part of a multidisciplinary team.Strong computer skills, including Microsoft Office and electronic documentation systems.Ability to maintain confidentiality and handle sensitive client information professionally.Spanish/English bilingual skills preferred.Must be able to travel throughout all five NYC boroughs.Benefits17–27 PTO days, 10 sick days, 3 personal days, 12 holidays, health, dental and vision coverage, 403(b), transit subsidy, and tuition reimbursement.
