Certified Diabetes Care and Education Specialist (CDCES)

About the RoleWe are building a virtual endocrinology practice focused on type 2 diabetes. The CDCES is central to that model — you own the education, behavior change, and between-visit management that determines whether patients actually get better. You will work alongside endocrinologists and nurse practitioners on a shared panel, and you will help shape how the program runs as we scale.What You'll Do•  Deliver individual DSMES visits (initial and follow-up) via telehealth, covering medication use, glucose monitoring, nutrition, activity, sick-day rules, and complication prevention•  Review CGM and BGM data between visits and surface actionable patterns to the prescribing clinician•  Build and maintain individualized care plans with measurable goals; document in EHR•  Support insulin and non-insulin titration understanding protocols and clinician direction•  Manage the between-visit touchpoint cadence for your panel, including CCM/RPM-eligible outreach•  Provide device training and troubleshooting (CGM, pens, pumps as applicable)•  Partner with the clinical team on protocol development, patient education materials, and workflow design•  Contribute to outcomes tracking — A1C, time in range, engagement, and program-level reporting•  Precept and onboard future CDCES hires as the team growsWhat You Need•  Active CDCES credential in good standing (CBDCE)•  Active, unrestricted New York license as an RN, RD/RDN, RPh, NP, PA, or other CBDCE-eligible discipline; willingness to obtain additional state licensure as we expand•  3+ years of direct diabetes care and education experience, with meaningful type 2 volume•  Comfort working independently in a fully remote, video-first care model•  Strong documentation habits and EHR fluencyNice to Have•  Prior telehealth or digital health experience•  Familiarity with CCM, RPM, or RTM billing requirements•  Additional state licensure (multi-state or compact)•  Bilingual English/Spanish
