About CompanianCompanian provides virtual care coordination for seniors. Each patient is paired with a dedicated Care Advocate who handles the work that happens between doctor visits: records transfers, appointments, medication support, insurance navigation, and connections to community resources. Our care management programs are physician-referred and covered by Medicare. We partner directly with practices to extend their care teams and make sure no senior falls through the cracks between visits.We're scaling nationally, and we're looking for a physician leader with active licensure across all 50 states and deep care management experience to set the clinical standard as we grow.The RoleAs CMO, you'll own the clinical design, quality, and oversight of Companian's care management programs nationwide. You'll provide physician supervision for patients in every state we serve, lead our clinical organization (Chief Nursing Officer → Care Directors → Care Leaders → Care Advocates), and be the medical voice of Companian with our partner practices.What You'll DoLead the clinical design and delivery of Medicare care management programs, including CCM, PCM, APCM, RPM, and PIN/CHIProvide physician oversight and general supervision for care management services in all 50 states, in line with CMS requirementsBuild and approve care management protocols: care plans, care transitions, medication reconciliation support, chronic condition management, and escalation pathwaysLead and mentor the Chief Nursing Officer and the nurse practitioner–led care leadership teamOwn clinical quality, patient safety, and outcomes measurement across programs, and turn the data into improvementsAct as a peer-to-peer partner to referring physicians and join key client development conversationsGuide clinical oversight of our technology, including AI-assisted documentation and care recommendationsPartner with leadership on compliance (HIPAA, CMS, state regulations) and national expansionWhat You BringMD or DO, board-certified in Internal Medicine, Family Medicine, or GeriatricsActive, unrestricted medical licenses in all 50 states, or the ability to obtain them quickly (Interstate Medical Licensure Compact eligibility a plus)5+ years of hands-on leadership in care management: CCM, PCM, APCM, RPM, transitional care, or population health programsStrong working knowledge of Medicare care management billing, documentation, and supervision requirements7+ years of clinical practice, including 3+ years in a clinical leadership roleA collaborative leader who can build structure in a fast-moving startup and earn the trust of practicing physiciansNice to HaveExperience serving as supervising physician for a national or multi-state telehealth or care management programGeriatric or chronic care focusExisting relationships with primary care, concierge, or specialty practices
