Team members work collaboratively with community partners, schools, and families to build healthier, more sustainable futures for those they serve.Role Description The Clinical Coordinator must be a LCSW-C. This is a full-time position based in Baltimore, MD with a hybrid work arrangement, combining on-site responsibilities with some work-from-home flexibility. The role involves coordinating clinical services and schedules, supporting treatment planning, and ensuring that therapeutic, psychiatric rehabilitation, and behavior services are delivered in accordance with agency standards and regulatory requirements. Day-to-day tasks include reviewing referrals, maintaining client records, tracking service utilization, and communicating with clinicians, case managers, families, and community partners to support continuity of care. The Clinical Coordinator will assist with data collection for program evaluation, help identify service gaps, and support quality improvement initiatives. This position also involves participating in team meetings, supporting documentation accuracy, and helping new staff understand program workflows and expectations.QualificationsStrong Communication skills, including clear documentation and professional interaction with clients, families, and multidisciplinary teams.Knowledge of Medicine and Patient Care principles relevant to behavioral health, psychiatric rehabilitation, and community-based services.Familiarity with Critical Care Medicine concepts and their implications for coordinating care with medical and behavioral health providers.Ability to conduct and interpret basic Research related to program outcomes, evidence-based practices, and quality improvement.Experience in mental or behavioral health settings, such as outpatient, school-based, residential, or community programs.Strong organizational, time management, and problem-solving skills, with attention to detail in record-keeping and compliance.Proficiency with electronic health records and standard office software; comfort working in a hybrid environment.Relevant education in social work, psychology, counseling, nursing, public health, or a related field; licensure or certification in a clinical or human services discipline is preferred but not required depending on experience.
