Clinical Pharmacist (Woburn, MA)ResponsibilitiesReview and verify medication orders for accuracy, safety, dosing, and compatibility prior to dispensing.Evaluate patient suitability for home infusion therapy and support safe transitions from acute care settings to home-based treatment.Develop and monitor individualized care plans, including therapeutic goals, outcomes tracking, and communication with prescribing providers.Serve as a clinical medication resource for healthcare teams regarding drug interactions, side effects, administration, storage, and dosing considerations.Educate patients and caregivers on medication therapy, infusion procedures, and safe home management practices.Prepare, compound, and dispense medications in accordance with clinical standards and regulatory requirements.Identify medication-related risks, including adverse reactions, therapy duplication, and opportunities for optimization; collaborate with prescribers on recommendations.Document patient progress, clinical interventions, and care coordination activities while maintaining compliance with documentation standards.Coordinate communication among providers, nursing teams, reimbursement staff, and home health partners to promote continuity of care.Provide guidance on infusion equipment use and medication administration processes.Maintain patient confidentiality and ensure compliance with HIPAA and applicable privacy regulations.Required SkillsBachelor of Science degree from an accredited school of pharmacy.Registered Pharmacist, in good standing, with a valid license.Preferred SkillsDoctorate of Pharmacy degree.Knowledge of HIPAA Privacy and Security Regulations.Knowledge of JCAHO accreditation process and overall JCAHO standards.Working / practical knowledge of microcomputer systems including patient documentation systems, Microsoft Office (Word, Excel, Outlook), automated pharmacy systems, and IV Laminar flow Hood.Two (2) years’ experience in the field of intravenous admixing of all types required, with an understanding of stability and compounding.Schedule/ShiftM-F, normal business hours, ideally will match most staffed schedule for training early on. Includes a weekend rotation every four weeks (Sat/Sun 9:00 AM–5:00 PM) with a corresponding weekday off. - 40 hours
