About the RoleThe Clinical Pharmacist will work across Worcester City PCN and its member practices, providing expert pharmaceutical advice and clinical leadership to improve the quality, safety and effectiveness of prescribing.You will play a central role in delivering medicines optimisation services, supporting patients with long-term conditions, conducting structured medication reviews, improving patient safety and helping practices achieve key quality standards and contractual requirements.Working as part of a highly skilled multidisciplinary team, you will collaborate closely with GPs, nurses, pharmacy technicians, community pharmacists, hospital colleagues and wider healthcare professionals to ensure patients receive the best possible care.This role offers variety, challenge and significant opportunities for professional growth. Clinical Pharmacist will play a vital role in helping us achieve these objectives.Key ResponsibilitiesStructured Medication ReviewsYou will provide comprehensive structured medication reviews for patients with complex needs, including:Patients with multiple long-term conditions.Patients experiencing problematic polypharmacy.Care home residents.Frail older adults.Housebound patients.Patients prescribed high-risk medications.Patients with mental health conditions.Patients requiring optimisation of long-term therapies.Reviews will focus on enhancing medicines safety, improving adherence, reducing unnecessary polypharmacy and ensuring treatment remains evidence-based and patient-centred.Medicines OptimisationYou will lead and support medicines optimisation activities across the PCN through:Identifying opportunities to improve prescribing quality.Supporting implementation of NICE guidance.Reviewing high-cost prescribing areas.Delivering prescribing efficiency projects.Supporting formulary adherence.Monitoring prescribing trends.Promoting evidence-based treatment pathways.Supporting local and national prescribing priorities.Long-Term Condition ManagementThe role will involve proactive support for patients living with conditions such as:Hypertension.Diabetes.Asthma.COPD.Cardiovascular disease.Osteoporosis.Mental health conditions.Chronic kidney disease.Rheumatological conditions.Chronic pain.You will work alongside multidisciplinary colleagues to optimise treatment plans and improve patient outcomes.Patient SafetyPatient safety is fundamental to this role.You will:Implement MHRA safety alerts.Action medicine recalls and supply notifications.Monitor high-risk prescribing.Undertake prescribing safety audits.Review incidents and significant events relating to medication.Develop safer prescribing systems.Reduce avoidable medicines-related harm.Transfer of CareYou will support patients moving between care settings by:Completing medicines reconciliations.Reviewing hospital discharge information.Communicating with secondary care colleagues.Supporting seamless transitions between services.Reducing avoidable medication discrepancies.Education and Clinical SupportThe successful candidate will provide expert advice and guidance to:General Practitioners.Advanced Clinical Practitioners.Practice Nurses.Healthcare Assistants.Pharmacy Technicians.Social Prescribers.Care Coordinators.Community pharmacists.You will also contribute to educational programmes and support workforce development across the PCN.What We degree.Current GPhC registration.Completion of CPPE Primary Care Pharmacy Education Pathway (or equivalent).Minimum two years post-registration pharmacy experience.Experience working within primary care or general practice environments.Evidence of continuing professional development.Desirable QualificationsIndependent Prescribing Qualification.Membership of the Royal Pharmaceutical Society.Experience of QOF, QPMS, DES and LES delivery.Experience leading quality improvement initiatives.Experience supervising pharmacy professionals.
