Team members collaborate across disciplines to support a mission-driven environment grounded in compassion, equity, and community engagement.Role Description The Clinical Pharmacist is a full-time, on-site role based in Elkhorn, WI. This role is responsible for reviewing and verifying medication orders, providing pharmaceutical care, and collaborating with medical providers to ensure safe and effective therapy for patients. Daily tasks include performing patient medication counseling, conducting medication reconciliation, preparing and dispensing medications. The Clinical Pharmacist will monitor treatment plans, identify potential drug interactions, support medication access for uninsured and low-income patients, and maintain accurate documentation in accordance with clinic policies and regulatory standards. The role also involves educating patients and staff on appropriate medication use, contributing to quality improvement initiatives, and actively supporting the clinic’s mission to serve underserved populations.QualificationsDemonstrated proficiency in Pharmacy Practice and general Pharmacy operations, including dispensing and medication safety.Experience providing patient-centered Pharmaceutical Care, with a strong focus on counseling and therapy optimization.Solid foundation in Medicine as it relates to pharmacotherapy, drug interactions, and evidence-based treatment guidelines.Active and unrestricted Pharmacist license in the state of Wisconsin (or eligibility to obtain), and graduation from an accredited school of pharmacy (PharmD or equivalent).Strong communication and interpersonal skills, with the ability to work collaboratively in a multi-disciplinary, community-based setting.Commitment to serving uninsured, low-income, and underserved populations, with cultural humility and respect for diverse backgrounds.Experience in a clinical or ambulatory care setting is preferred; prior work in free clinics or community health environments is a plus.Ability to maintain accurate records, follow clinic protocols, and adhere to applicable laws, regulations, and professional standards.
