We are seeking a hybrid Clinical Pharmacist to join a growing pharmacy team within a healthcare organization in the Pittsburgh area. In this role, you will verify prescriptions, review medication therapy, and counsel patients, serving as a trusted clinical resource for patients and their providers. This position is a full time, direct hire opportunity with a hybrid schedule that includes regular remote work.This role is well suited for a pharmacist who wants to practice at the top of their license and build ongoing relationships with the patients they serve. Pharmacists from hospital, retail, ambulatory care, and infusion backgrounds are encouraged to apply, and training on internal workflows and systems is provided. The ideal candidate is a strong communicator, clinically curious, and excited to join a pharmacy team during a period of growth.

Responsibilities

Verify prescriptions for accuracy, appropriateness, and safety before dispensingReview patient profiles for drug interactions, allergies, dosing, and duplicate therapyProvide medication therapy management and make clinical recommendations to prescribersCollaborate with physicians, nurses, and care teams to develop and support patient treatment plansContact prescribers to clarify orders and resolve clinical or prescription issuesCounsel patients and caregivers on how to take their medications, what to expect, and how to manage side effectsFollow up with patients by phone to support adherence and monitor progress toward therapy goalsHelp patients work through prior authorizations, insurance coverage questions, and other access barriersAnswer medication questions from patients, caregivers, and providersDocument clinical interventions, counseling, and patient interactions accuratelySupport pharmacy quality, safety, and compliance with state and federal regulations

Qualifications

Doctor of Pharmacy (PharmD) from an accredited pharmacy programActive Pennsylvania pharmacist license in good standingPharmacist experience in a hospital, retail, ambulatory care, infusion, or similar settingStrong clinical knowledge and patient counseling skillsWillingness to obtain additional state pharmacist licenses as neededMust reside within commuting distance of Pittsburgh, PASchedule:Full time, direct hireMonday through Friday, standard daytime hours, Eastern TimeHybrid schedule with regular remote work following an initial on site training periodLimited, occasional on call coverage, typically handled by phone from home, with an on call stipend and additional pay for time worked.
