Each contribution of time, resources, or expertise helps move patients closer to breaking the sickle cell cycle and achieving better health outcomes.Role Description The Community Health Worker is a full-time, on-site role based in Macon, GA. This role focuses on providing direct community outreach, education, and support to individuals and families affected by sickle cell disease and related conditions. Day-to-day responsibilities include conducting outreach in community settings, connecting clients to healthcare and social services, and helping them navigate appointments, screenings, and follow-up care. The Community Health Worker will assist with health education workshops, support groups, and community events, and will maintain accurate records of client interactions and services provided. This position works closely with clinical and program staff to coordinate care, identify community needs, and support initiatives that improve access to resources and health outcomes.QualificationsExperience in community outreach, client advocacy, and health education in underserved or diverse communities.Knowledge of basic public health concepts, care coordination, and navigating healthcare and social service systems.Strong communication, active listening, and interpersonal skills to build trust and rapport with clients and community partners.Organizational and time-management abilities, including accurate documentation and follow-up on referrals and services.Comfort working in community settings, including homes, clinics, and local organizations, and collaborating in a multidisciplinary team.High school diploma or equivalent required; additional training or certification in community health work, social services, or public health is preferred.Experience working with individuals with chronic health conditions, especially sickle cell disease or hematology-related disorders, is a plus.Basic computer skills, including use of email, data entry, and electronic records; ability to learn organizational systems and tools.Commitment to health equity, cultural humility, and non-discriminatory, person-centered care.
