Agape Hospice & Palliative Care is dedicated to helping patients and families navigate this time with dignity, respect, and peace.Role Description The Community Liaison is a full-time, on-site role based in Los Angeles, CA. This role focuses on building and maintaining relationships with community organizations, healthcare providers, and families to increase awareness of hospice and palliative care services. Day-to-day responsibilities include coordinating community outreach activities, attending local events, giving presentations, and meeting with referral sources to explain available services. The Community Liaison will respond to inquiries, provide information and resources, and support families in understanding care options, working closely with clinical staff to ensure a smooth transition into services. The position also involves tracking outreach efforts, maintaining accurate records, and contributing to strategies that strengthen Agape Hospice & Palliative Care’s presence in the community.Qualifications Candidates should possess strong Community Engagement and Community Outreach skills, with experience building relationships across diverse communities.Candidates should possess excellent Communication and Customer Service skills, including active listening, empathy, and clear, professional verbal and written communication.Candidates should possess knowledge or experience related to Home Care or hospice and palliative care settings, or a strong interest in learning about end-of-life care services.Beneficial qualifications include prior experience in healthcare, social services, or nonprofit outreach; familiarity with Los Angeles and Orange County communities; and the ability to collaborate effectively with interdisciplinary care teams.Additional preferred qualifications include a valid driver’s license for local travel, proficiency with basic office software, and a high school diploma or equivalent (bachelor’s degree in a related field is a plus).
