Team members benefit from working within an integrated network that emphasizes quality, safety, and compassionate care for diverse patient populations.Role Description The Diabetes Educator is a full-time, on-site role based in Walnut Creek, CA, responsible for providing individualized and group education to pediatric patients and their families on diabetes management. Day-to-day responsibilities include assessing learning needs, developing and implementing education plans, and coaching patients on blood glucose monitoring, insulin administration, nutrition, and lifestyle modification. The role collaborates closely with endocrinologists, dietitians, nurses, and other members of the care team to support safe and effective diabetes care, including participation in care conferences and interdisciplinary planning. The Diabetes Educator also maintains accurate documentation, contributes to quality improvement initiatives, and stays current with clinical guidelines and best practices in pediatric diabetes care. This position requires active engagement with patients and caregivers, providing empathetic support, clear communication, and culturally sensitive education.Qualifications Strong foundation in Dietetics, including medical nutrition therapy and meal planning for individuals living with diabetes.Demonstrated expertise in Diabetes Management and Diabetes Care, with the ability to translate complex clinical information into practical, age-appropriate guidance.Certification as a Certified Diabetes Educator (or Certified Diabetes Care and Education Specialist) and comprehensive knowledge of Diabetes-related clinical standards.Experience working with pediatric populations and families, ideally in an inpatient or outpatient endocrinology or pediatric setting.Relevant clinical licensure (e.g., RN, RD/RDN, or other applicable healthcare credential) and compliance with state regulatory requirements.Excellent communication, teaching, and motivational interviewing skills, with the ability to build trust and rapport across diverse backgrounds.Ability to work collaboratively within an interdisciplinary team, manage multiple priorities, and maintain accurate clinical documentation in electronic health records.Commitment to ongoing professional development and staying current with evidence-based practices in pediatric diabetes education and care.
