In this position, you will empower individuals with the skills, tools, and confidence needed to manage diabetes and make long-term lifestyle changes. You will work closely with patients, caregivers, and the healthcare team. You will be at the forefront of ensuring the safe and effective use of medications, working closely with healthcare professionals and patients to optimize treatment outcomes. If you have a passion for patient safety, a keen eye for detail, and thrive in a fast-paced environment, we invite you to apply and become a vital part of our mission to provide exceptional healthcare services.Your responsibilities include the following:Conducting Diabetes Care and Education (DCES) participant assessments using the AADE7 frameworkDeveloping measurable goals, teaching plans, and individualized learning objectivesProviding education using interactive and skill-based methods to increase engagement and understandingDocumenting patient assessments, learning plans, progress, and evaluations outcomesPerforming follow-up assessments and track participant progress toward defined goalsReferring patients to appropriate health providers as neededCommunicating relevant findings and updates to the primary care provider and healthcare teamParticipating in developing program and teaching materialsSupporting program accreditation requirements and participate in accreditation surveysEngaging in continuous quality improvement initiatives for Diabetes Care and Education Specialist (DCES)Maintaining required annual continuing education specific to diabetes and behavioral change strategiesStaying current with advancements in diabetes management, technology and educational best practicesRequired Qualifications:Active RN LicenseAt least 1 year experience with DCES programsAt least 1 year experience developing therapeutic relationships and motivating participantsStrong understanding of diabetes treatment and educationAbility to present education contentStrong communication and teaching skillsPreferred Qualifications:Certified Diabetes Educator (CDE) or experience working in diabetes education preferredAt least 3 years experience with DCES programsAt least 3 years experience developing therapeutic relationships and motivating participants
