The organization provides education, resources, and support for adults and children living with diabetes in Southern New Mexico. Its programs focus on practical guidance, prevention, and ongoing management to help community members improve quality of life. Team members collaborate with healthcare partners, schools, and community groups to expand access to diabetes education and support services.Role Description The Diabetes Educator is a full-time, on-site role based in the United States, responsible for delivering diabetes self-management education and support to adults and pediatrics. Day-to-day tasks include assessing individual needs, developing personalized education plans, and providing instruction on blood glucose monitoring, medication adherence, nutrition, and physical activity. The role involves leading group classes, workshops, and community outreach events, as well as creating educational materials tailored to diverse audiences. The Diabetes Educator collaborates with healthcare providers, families, and community organizations to coordinate care, track participant progress, and ensure culturally sensitive, evidence-based education. Additional responsibilities include maintaining accurate documentation, staying current with diabetes care guidelines, and contributing to program development and evaluation.Qualifications Candidates should possess strong expertise in Diabetes Management and Diabetes Care, with the ability to apply current clinical guidelines to diverse age groups.Candidates should possess effective communication and teaching skills, including the ability to explain complex information clearly and adapt education to different literacy levels.Candidates should possess strong interpersonal skills, cultural competency, and a commitment to non-discriminatory, patient-centered care.Candidates should possess relevant professional licensure (e.g., RN, RD, or other health-related credential) and prior experience in diabetes education or chronic disease management; bilingual abilities are a plus.Bachelors degree in a health related science required.
