We are passionate about exceeding expectations for our clients and their families, offering care that is both personalized and meaningful.CompassCare is excited to be growing its team and is now looking for an experienced Registered Nurse to serve as our Director of Clinical Services — the organization’s primary clinical authority and the trusted resource clients, families, caregivers, and leadership turn to for safe, high-quality, client centered care.This is a hands-on leadership role spanning clinical oversight, quality assurance, care, coordination, education, and program development, based in Fairfield County with regular work across Westchester County and Manhattan. What You’ll DoClient Assessment & Clinical OversightConduct comprehensive initial assessments and ongoing client reassessments, evaluating physical, cognitive, emotional, and environmental factors.Provide clinical oversight for clients experiencing significant changes in condition, safety concerns, injuries, hospitalization, or complex care needs care. Planning & CoordinationDevelop, implement and update individualized care plans, and coordinate services across physicians, therapists, discharge planners, social workers, and hospice providers.Coordinate client transitions between home, hospital, rehabilitation, assisted-living, skilled nursing, and hospice settings, including discharge planning and follow up. Family & Caregiver SupportServe as a clinical advisor to clients and families, providing education on changing care needs, disease progression, and care options.Serve as the primary clinical resource for caregivers and office staff on care, documentation, dementia care, hospice care, and other clinical issues. Quality Risk & Clinical EducationReview and investigate clinical incidents and safety events (falls, wandering, medication concerns, skin integrity, hospital transfers), and lead follow up and corrective action.Monitor clinical outcomes, conduct quality reviews and audits and drive continuous improvement initiatives.Partner with a talent team to build training resources and competency programs for caregivers and clinical staff. Operations, Policy & LeadershipProvide clinical guidance to the Administrator-on-Duty team, including triage processes and escalation procedures.Participate in clinical review and intake assessments for prospective clients, including home safety evaluations and staffing recommendations.Develop and maintain clinical policies, procedures, and best practice guidelines; contribute to strategic planning alongside operations, client services, and executive leadership. QualificationsWhat You BringRequired ExperienceActive Registered Nurse LicenseBachelor’s degree in nursing or related healthcare fieldMinimum five years of Nursing experience, Including at least three years in Leadership, Case Management, Hospice, Geriatric Home Care, Or Related Clinical Work.Strong Knowledge of Aging, Chronic Illness, Dementia Care, Care Coordination and Quality ManagementExcellent Communication, Organizational and Interpersonal Skills Preferred ExperienceMaster’s degree in nursing healthcare administration or Related Field.Experience in Private Duty Home Care, Concierge Care, Hospice, Geriatric Care, Management or Quality Assurance Leadership.
