Team members work in an environment that values collaboration, service, and improving health outcomes for underserved populations.Role Description The Family Physician is a full-time, on-site role based in Terre Haute, IN, responsible for delivering high-quality primary care to patients of all ages in a community health setting. Day-to-day responsibilities include conducting patient assessments, diagnosing and managing acute and chronic conditions, providing preventive care and health education, and coordinating referrals to specialty and ancillary services as needed. The physician will manage health care needs within the scope of family medicine, review and update treatment plans, and maintain accurate, timely medical records. This role involves close collaboration with a multidisciplinary team, participation in quality improvement initiatives, and adherence to FQHC and PCMH standards and guidelines to ensure comprehensive, equitable care. Participation in on-call coverage in collaboration with partner physicians. QualificationsDemonstrated proficiency in Primary Care and Family Medicine, with the ability to provide comprehensive care across the lifespan.Commitment to Community Health, including serving diverse and underserved populations in a team-based environment.Medical degree (MD or DO) from an accredited institution and completion of an accredited Family Medicine residency program.Current, unrestricted license to practice medicine in the state of Indiana (or eligibility to obtain), and board certification in Family Medicine.Strong interpersonal, communication, and documentation skills, with the ability to work collaboratively in a multidisciplinary team.
