About Salud Healthcare & the RoleSalud Healthcare is a tech-forward services organization that helps private practices transition to alternative payment models. The Health Coach II will work directly with members — the patients of our affiliated practices — helping them manage chronic conditions and take the next step in their care. The role plays a key part in how we support the providers we serve and the communities they care for.Position SummaryReporting to our AVP, Patient Engagement, this role is responsible for engaging members with chronic disease and high-risk medical conditions. The work centers on structured coaching conversations, built on motivational interviewing (MI), that bridge the member to a clear call to action around their health, such as scheduling a preventive primary care visit with their doctor.Job ResponsibilitiesCoach members on chronic disease self-management (diabetes, hypertension, and cardiovascular disease), as well as nutrition and behavior change.Conduct health assessments, develop personalized goals and wellness plans, and follow up on progress.Coordinate care plan logistics across providers, specialists, facilities, and community-based organizations.Assess members across the eight dimensions of wellness: emotional, environmental, financial, intellectual, occupational, physical, social, and spiritual.Set goals that are about behavior and belong to the member — not outcome goals or instructions from a care-gap list.Explain things in plain language, using teach-back and everyday words (e.g., “high blood pressure” instead of “hypertension”).Handle objections using the MI framework: fill information gaps, reflect sustain talk, and treat pushback as a relationship problem.Put member safety first.Encourage annual preventive care visits and a complete review of conditions and medications with the member’s doctor. Never suggest a diagnosis.Conduct outreach by phone, SMS, and video, and document every contact accurately in Nexus, our care platform.Pull the “Andon cord”: take a broken script, referral path, or letter out of use pending review.Take part in 15-minute daily team standups.Qualifications1–2 years of experience in health coaching or chronic-condition patient education.Bachelor’s degree in health science, nutrition, or a related field.Strong motivational interviewing and coaching skills, grounded in behavior-change principles, with empathy and patience.Tech-forward, with EHR proficiency and comfort using AI-assisted tools.PreferredBilingual in English and Spanish.Active healthcare certification: CNA, CMA, CHC, or equivalent.Working knowledge of HEDIS measures, HCC coding, and value-based care principles.
