As we continue to grow and reach new heights, we're looking for passionate individuals who share our vision and values.Job Summary:Provide clinical advice and education to people living with, or who are at risk of developing diabetes. Support them on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing, overcoming challenges, and leading their healthiest life possible.2. Primary Responsibilities:Technical:1. Assisting people with diabetes, their families and care giver to gain the knowledge, skills, motivation and confidence to manage their condition.2. Educate people on their diabetes, including complications with diabetes and its relation to other health conditions.3. offer advices and support around lifestyle changes that support diabetes management;4. Give guidance on medication management safety (actions, timing, side effects) including the use of insulin therapy (e.g. correct injection technique, checking injection sites).5. Provide education on the self-monitoring of blood glucose levels including technique and interpretation of results.6. support goal setting and the implementation of strategies to achieve health goals;7. Encourage collaboration between other health providers involved in the diabetes health care team, such as general practitioners (GPs), specialist medical practitioners (endocrinologists/diabetologists), dietitians, physiotherapists, podiatrists, optometrists, pharmacists and others.8. Introduction, education, demonstration and installation of Continues Glucose Monitoring devices (CGM) and other devices like (Insulin pump) in patients on multiple daily Insulin injection aiming significant improvement in quality of life.9. Maintain knowledge of the latest information and technology in the field of diabetes10. Monitor and document patient progress11. Perform other professional duties as assigned.Education / Professional Qualifications:3.1. Education Degree: Bachelor’s degree in Nursing or diabetes education diploma.3.2. Years of Experience: Minimum 5 years of experience within a related field.
