This position is responsible for coordinating home care services, including organizing client assessments, developing care plans, and monitoring ongoing needs. Day-to-day tasks include communicating with clients, families, and clinical staff; scheduling and tracking visits; and ensuring services adhere to care plans and regulatory requirements. The Healthcare Coordinator maintains accurate documentation, responds to service inquiries, and addresses client concerns in a timely, professional manner.
