Each year, the organization serves over 40,000 unique patients through more than 100,000 encounters, providing services such as family practice, obstetrics and gynecology, dentistry, podiatry, pharmacy, laboratory, and X-ray.Role Description The Healthy Start Care Coordinator is a full-time, on-site role based in Orlando, FL, responsible for coordinating comprehensive maternal and child health services for patients enrolled in the Healthy Start program. This role includes conducting intake assessments, screening for risk factors, and developing individualized care plans to support healthy pregnancies, births, and early childhood outcomes. The Care Coordinator will provide education, referrals, and linkage to community resources, and will collaborate closely with medical, dental, behavioral health, and social service teams to ensure continuity of care. Daily responsibilities also include maintaining accurate documentation, tracking patient progress, and following up to address barriers to care such as transportation, insurance, or social determinants of health. The individual in this role will engage patients and families in a culturally sensitive manner, promote preventive care, and support True Health’s mission to improve health outcomes in underserved communities.Qualifications Candidates should possess strong Interpersonal Skills and Customer Service abilities to build trust, support patients and families, and deliver compassionate, patient-centered care.Candidates should possess effective Communication skills to clearly explain care plans, provide health education, coordinate with clinical teams, and document encounters accurately.Candidates should possess foundational knowledge in Medicine and, where applicable, exposure to Critical Care Medicine to understand maternal and child health needs and recognize when to escalate care.Experience in care coordination, case management, or community health settings, preferably serving diverse and underserved populations.Ability to work collaboratively in an interdisciplinary team, manage multiple cases, and prioritize tasks in a fast-paced clinical environment.Proficiency with electronic health records and basic computer applications for documentation, scheduling, and reporting.High school diploma or equivalent required; additional training or education in nursing, social work, public health, or a
