We have physician opportunities available across three locations in the Chicago area: Cicero (60802), Chicago (60631), and Chicago (60641).This is an excellent opportunity for a physician seeking competitive compensation, a weekday outpatient schedule, and the opportunity to build lasting relationships with patients in a collaborative, community-focused healthcare environment.Compensation & Incentives$250,000–$300,000+ base salary, based on experience and qualifications.Up to $100,000 in potential bonuses.$10,000 sign-on bonus: $5,000 after 30 days of employment and $5,000 after 6 months of employment.Medical, dental, and vision insurance.401(k) retirement plan.Paid time off and paid holidays.CME allowance and licensing support.Malpractice coverage.What You'll DoProvide comprehensive outpatient primary care within the scope of Family Medicine or Med-Peds.Diagnose and manage acute and chronic medical conditions.Perform annual wellness visits, preventive screenings, and routine physical examinations.Develop individualized treatment plans focused on prevention, chronic disease management, and long-term wellness.Educate patients on lifestyle modifications, preventive health, and disease management.Build strong, lasting relationships with patients and families.Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health professionals, and specialty teams.Coordinate referrals, diagnostic services, and follow-up care.Maintain accurate and timely electronic health record (EHR) documentation.Follow applicable clinical, regulatory, and quality standards while supporting value-based care initiatives.Required QualificationsMD or DO degree from an accredited medical school.Board Certified or Board Eligible in:Family Medicine, orInternal Medicine-Pediatrics (Med-Peds).Active Illinois medical license or ability to obtain one before starting.DEA registration and applicable controlled substance authorization, or ability to obtain them as required.Strong commitment to patient-centered, high-quality care.Preferred QualificationsSpanish or Polish fluency is preferred but not required.Experience serving diverse or underserved patient populations.Experience in an outpatient primary care setting.Familiarity with value-based care, population health, or chronic disease management.Why Join EPIC Health?Competitive base compensation and substantial bonus potential.Weekday outpatient schedule.Collaborative, integrated care model with clinical and operational support.Opportunity to make a meaningful difference in the community.Professional growth and potential leadership opportunities.Flexibility to be considered for available physician openings across our three Chicago-area locations.Ready to Join EPIC Health?We're hiring urgently!
