This role is responsible for coordinating and scheduling home health visits, ensuring that clinicians are assigned appropriately based on patient needs, availability, and location. The coordinator receives and processes new referrals, gathers and verifies patient information, and confirms insurance and eligibility as needed. Daily activities include maintaining accurate schedules in the system, updating appointments and visit changes, and communicating promptly with clinicians, patients, families, and referral sources. The role also involves tracking documentation, following up on missing information, prioritizing urgent requests, and collaborating with the clinical team and management to support efficient operations and high-quality patient care.Qualifications Proficiency in scheduling, coordinating appointments, and managing calendars in a health care or similar setting.Strong intake, data entry, and documentation skills, with attention to accuracy and compliance.Effective communication and interpersonal skills for working with patients, families, clinicians, and referral partners.Solid organizational and time management abilities, including prioritizing multiple tasks and meeting deadlines.Comfort with electronic health records , office software, and basic computer applications.Experience in home health, hospital, or outpatient care coordination is preferred.Ability to work on-site in Palm Coast, FL in a fast-paced environment while maintaining professionalism and discretion.High school diploma or equivalent required; postsecondary education in health care administration or related fields is a plus.
