Professionally trained staff are supervised by licensed clinicians and offer services to all community members, including speakers of multiple languages such as Chinese, Japanese, Korean, Tagalog, Vietnamese, Spanish, and others. PACS offers individual, couple, family, and group counseling for a wide range of needs, including depression, anxiety, trauma, grief, substance use concerns, and school-related issues. Clients also receive psychiatric consultation, medication support, and assistance with case management needs such as benefits enrollment, school communication, vocational support, and linkage to legal and immigration services. The organization emphasizes culturally responsive care, conflict resolution, and support during major life changes such as divorce, job loss, and relocation.Role Description The Home Visitor is a full-time hybrid role based in Los Angeles, CA, with some work-from-home flexibility. This role involves conducting home visits to support individuals and families, assessing needs related to mental health, social services, and basic resources, and connecting clients to appropriate PACS programs and external community resources. The Home Visitor documents client interactions, maintains accurate records, and collaborates with clinicians, case managers, and other staff to develop and follow through on care plans. Day-to-day tasks include providing psychoeducation, coaching clients on communication and conflict resolution, assisting with appointments and referrals, and offering culturally sensitive support to monolingual and limited English-speaking clients. The Home Visitor also participates in team meetings, trainings, and outreach activities to strengthen family engagement and community relationships.Qualifications Strong interpersonal, communication, and active listening skills, with the ability to build trust and rapport with diverse individuals and families.Knowledge of basic mental health concepts (e.g., depression, anxiety, trauma), and comfort working under the guidance of licensed clinicians.Experience in social services, case management, or community outreach, including connecting clients to benefits, schools, vocational, legal, and immigration resources.Cultural humility and sensitivity when working with multilingual and multicultural communities; proficiency in an additional language commonly served by PACS is highly beneficial.Strong organizational and documentation skills, including maintaining accurate records and managing appointments across multiple clients.Ability to work independently in the field and remotely, while collaborating effectively as part of an interdisciplinary team.Basic computer literacy and comfort with electronic health records or case management systems.High school diploma or equivalent required; associate or bachelor’s degree in social work, psychology, human services, or related field preferred.Valid driver’s license, reliable transportation for home visits, and willingness to travel
