Job Title: Care Team Coordinator | Hospice Company: Superior Hospice Location: Golden Valley, MN Schedule: Full-Time | Monday–Friday, 8:00 a.m.–5:00 p.m. | One Remote Weekend Per Month Compensation: $25–$30 per hour, depending on experience You May Already Be Doing Half This Job Without Realizing It Are you the person at the nurses' station who keeps everything moving?

The person who knows which records are missing, which provider needs to be called, which patient is waiting, which order needs attention, and who on the clinical team needs an update? If you've worked as a Health Unit Coordinator, Unit Secretary, Medical Secretary, Referral Coordinator, Intake Coordinator, Admissions Coordinator, Patient Access Representative, Patient Care Coordinator, Clinic Coordinator, Medical Receptionist, or Healthcare Scheduling Coordinator, your experience could translate extremely well to hospice. Superior Hospice is looking for a Care Team Coordinator to help manage the communication, documentation, referrals, and coordination that allow patients and families to begin hospice care without unnecessary delays. You don't need to be the clinician providing bedside care. You need to be the person who can keep a lot of important moving pieces organized while remembering that there is a patient and family behind every referral. Why Your Healthcare Coordination Experience Matters Think about what you're already responsible for. You're comfortable with medical terminology. You've worked inside an EMR. You communicate with nurses, providers, patients, families, and other departments. You know how quickly priorities can change in healthcare, and you've learned how to stay organized when the phone is ringing and several people need something at once. Those skills matter tremendously in hospice. When a referral arrives, we need someone who can help move it forward, obtain missing information, communicate with the appropriate people, keep documentation organized, and make sure everyone understands what happens next. What You'll Do As our Care Team Coordinator, you'll become an important connection point between referral partners, patients, families, and our hospice clinical team. You'll help: Receive and review incoming hospice referrals. Gather medical records and required referral documentation. Communicate with hospitals, clinics, physicians, senior living communities, and other referral partners. Coordinate next steps in the hospice intake and admissions process. Enter and maintain accurate patient and referral information in the EMR. Communicate with patients and families during the referral process. Identify missing information and follow through until it is received. Coordinate with nurses and other clinical team members to support timely admissions. Help manage scheduling and changing priorities based on patient needs. Provide updates to referral partners and internal team members. Keep referral and admission documentation organized and accurate. Help identify and resolve barriers that could delay care. Your Current Job Title May Be Different. That's Okay. We're especially interested in people with healthcare experience as a: Health Unit Coordinator (HUC) | Unit Coordinator | Unit Secretary | Medical Secretary | Referral Coordinator | Referral Specialist | Intake Coordinator | Admissions Coordinator | Patient Access Representative | Patient Registration Representative | Patient Care Coordinator | Clinic Coordinator | Medical Receptionist | Scheduling Coordinator | Care Coordinator | Healthcare Administrative Coordinator Previous hospice intake or admissions experience is highly valued, but we are also interested in strong healthcare coordinators who have the right transferable experience and are ready to learn hospice. What Will Help You Succeed We're looking for someone who already understands the pace and responsibility that comes with coordinating healthcare. Strong candidates will bring experience with medical terminology, EMRs/EHRs, patient records, referrals, admissions, scheduling, insurance or eligibility information, healthcare documentation, and communication with clinical teams. You'll also need to be someone who follows through. If you're waiting on a document, you go get it. If a family needs an update, you communicate. If something could delay care, you don't simply leave it sitting in a queue for someone else. Just as importantly, you need compassion. Hospice referrals can happen during an incredibly difficult moment for a patient and family, and the person answering the phone or helping coordinate their next step can make that experience feel considerably less overwhelming.
