What You'll Do

Provide comprehensive primary care in an office/clinic setting, including wellness exams, preventive care, and chronic disease managementConduct in-home patient visits (typically 2–3 days per week), delivering personalized, home-based careEvaluate, diagnose, and treat a broad range of acute and chronic conditions across a diverse patient populationCoordinate care with on-site specialists (urology, cardiology, gastroenterology) and ancillary services (PT, mental health, pain management)Maintain accurate clinical documentation — scribe support available for providers who want extra help with EMRBuild deep, long-term patient relationships through ongoing follow-up and supportWhat You'll Bring:MD/DO with active or eligible NY licensureBoard certification preferred but not required (Internal Medicine or Family Medicine backgrounds welcome)Excellent interpersonal skills and a strong commitment to patient-centered careComfort with both in-office and in-home visit formatsRussian language skills required for this locationWhy You'll Love It Here:Competitive compensation: $210,000–$240,000Balanced hybrid schedule: Monday–Friday, mix of home visits and in clinicFull-time and part-time schedules both consideredMeaningful work building lasting relationships within the local community
