Applicants can expect to join a mission-driven organization that values integrity and patient-centered care.Role Description The Intake Coordinator – Home Infusion Referrals is a full-time, on-site role based in Chandler, AZ. This position is responsible for receiving, reviewing, and processing home infusion referrals from healthcare providers and facilities, ensuring all required documentation and orders are complete and accurate. Day-to-day tasks include verifying insurance benefits, obtaining prior authorizations, and coordinating with case managers, prescribers, and internal pharmacy and nursing teams to support timely initiation of therapy. The Intake Coordinator communicates with patients and caregivers to explain services, confirm demographic and insurance information, and address basic questions regarding the referral and intake process. This role also maintains organized records, updates electronic systems, and follows established protocols to support compliance, efficiency, and a positive experience for patients and partners.QualificationsStrong Communication skills, including clear verbal and written interaction with patients, caregivers, and clinical partners.Demonstrated Customer Service skills, with the ability to respond professionally, empathetically, and efficiently to inquiries and concerns.Experience with Case Management or care coordination, preferably in a healthcare, pharmacy, or home infusion setting.Working knowledge of Medical Terminology to understand orders, diagnoses, and clinical documentation.Insurance-related skills, including benefit verification, prior authorization processes, and familiarity with commercial and government payers.Attention to detail and strong organizational skills for managing multiple referrals, documents, and deadlines.Proficiency with computer systems, electronic health records, and standard office software.Ability to work collaboratively in an interdisciplinary team environment and adapt to changing priorities.High school diploma or equivalent required; post-secondary education in healthcare administration, medical office, or related field is preferred.Prior experience in a medical office, infusion pharmacy, or home health setting is highly beneficial.
