Our approach focuses on coordinated care, prevention, chronic disease management, and clear communication with patients and their families.Position SummaryMindify is seeking an experienced Internal Medicine Physician, Nurse Practitioner, or Physician Assistant to provide primary care services to adults at our Fair Lawn location.Fluency in both English and Spanish is required for this position. The provider must be comfortable conducting appointments, discussing diagnoses and medications, and explaining treatment plans in both languages.The position focuses on preventive care, acute medical concerns, chronic disease management, and patient education. The provider will also coordinate care with specialists, behavioral health clinicians, and other members of the healthcare team.ResponsibilitiesConduct medical evaluations and provide diagnosis and treatment for adult patientsManage acute and chronic conditions, including hypertension, diabetes, and cardiovascular diseasePerform annual physicals, wellness visits, and preventive screeningsDevelop and update individualized treatment plansOrder and interpret laboratory tests, imaging, and other diagnostic studiesCommunicate test results and follow-up recommendations to patientsPrescribe medications and monitor effectiveness and side effectsEducate patients about their conditions, medications, preventive care, and lifestyle changesDocument patient encounters accurately and on time in the EMRCoordinate care with specialists, behavioral health providers, and other healthcare professionalsParticipate in clinical meetings and quality-improvement activitiesFollow federal, state, and organizational requirementsProvide care in both English and SpanishRequired QualificationsOne of the following:MD or DO degree from an accredited medical schoolLicensed Nurse Practitioner with primary care or internal medicine experienceLicensed Physician Assistant with primary care or internal medicine experienceActive and unrestricted New Jersey professional licenseBoard certification or board eligibility in Internal Medicine for physician applicantsCurrent DEA registration and applicable prescriptive authorityProfessional fluency in spoken and written English and SpanishAbility to conduct complete patient visits in either language without an interpreterStrong clinical assessment, diagnostic, and patient-management skillsExperience using an EMRStrong interpersonal and communication skillsPreferred QualificationsAt least two years of outpatient internal medicine or primary care experienceExperience working with diverse or underserved populationsFamiliarity with value-based care and quality measuresCurrent BLS and ACLS certificationsCompensation and BenefitsSalary: $175,000–$325,000 per year, based on provider type, experience, credentials, qualifications, and schedulePaid time offPaid holidaysMalpractice insurance coverageCollaborative clinical environmentPhysical RequirementsAbility to sit, stand, and walk for extended periodsAbility to perform routine clinical examinations and proceduresAbility to use standard office and medical equipmentApplyPlease note: Professional fluency in both English and Spanish is required for consideration.Equal Opportunity EmployerMindify is an Equal Opportunity Employer.
