Apply & track with Apply EdgeThe organization delivers integrated programs linking housing, health, and social services for individuals in New York City who are experiencing homelessness or living on low incomes, as well as those managing severe mental illness, substance use disorders, or other special needs.Role & ResponsibilitiesThe Mental Health Clinician provides person-centered clinical and supportive services both on-site and out in the community to a designated caseload of clients. While this is an individual contributor role, the clinician is expected to provide leadership, facilitate team meetings, offer clinical guidance during crises, supervise student interns, and support overall management functions in the Supervisor's absence.
Key Responsibilities
Clinical Assessments & Evaluations: Conduct intake evaluations, mental status examinations, psychosocial assessments, and diagnostic evaluations utilizing the most recent DSM standards.Psychotherapy & Counseling: Provide evidence-based individual therapy, group therapy, wellness self-management sessions, and substance use counseling to foster psychiatric and medical stability.Service Planning & Case Coordination: Collaboratively develop, implement, and continuously revise person-centered service plans considering medical/mental health diagnoses, goals, employment/educational interests, medication regimens, and collateral contacts.Advanced Crisis Intervention: Utilize trauma-informed, person-centered crisis techniques; partner with emergency services and coordinate discharge planning and continuity of care with hospitals, rehabs, jails, and nursing homes.Medication & ADL Support: Assist clients with medication self-administration following agency protocol; support daily living activities including personal hygiene, apartment cleaning, donation processing, and pest mitigation (e.g., using the PackTite bed bug oven for client belongings).Benefits & Housing Navigation: Help clients identify, apply for, and maintain eligible local, state, and federal benefits (including providing rep-payee support); facilitate the complete supportive housing search, application, transition, and follow-up outreach process.Fieldwork & Community Outreach: Perform in-home apartment visits and escort clients to medical appointments, housing interviews, benefits offices, shopping trips, job interviews, school meetings, and court appearances.Administrative & Compliance: Maintain accurate, timely documentation in agency databases (including progress notes, ILPs, assessments, and benefit applications) in compliance with funder and agency guidelines. Lead policy reviews and internal in-service trainings in areas of expertise.
