Apply & track with Apply EdgeRemote Clinical PharmacistPosition OverviewWe are seeking a clinically focused pharmacist to support medication management initiatives and patient care programs in a remote setting. This role involves direct patient engagement, collaboration with healthcare providers, clinical documentation, and participation in quality improvement efforts aimed at optimizing medication outcomes.Key ResponsibilitiesPatient Care:Conduct telephonic medication reviews and patient consultations to identify opportunities for improved medication use and adherence.Provide education on medication therapies, chronic disease management, and available healthcare programs.Complete medication reconciliations and assess opportunities to improve therapeutic outcomes.Clinical Assessment:Evaluate patient information and clinical data to identify medication-related concerns and potential interventions.Maintain accurate and timely documentation of patient encounters, recommendations, and outcomes.Prepare clinical summaries and reports as needed to support patient care initiatives.Care CoordinationCollaborate with physicians, pharmacies, care teams, and patients to address medication-related issues and barriers to care.Support transitions of care and ensure timely follow-up on identified clinical needs.Medication OptimizationRecommend evidence-based interventions to improve medication utilization, adherence, and quality outcomes.Monitor therapy effectiveness and identify opportunities to enhance patient care.Workload ManagementManage a consistent volume of patient interactions while maintaining quality standards and productivity expectations.Contribute to departmental goals related to patient engagement, clinical outcomes, and documentation accuracy.Required QualificationsEducation & LicensureDoctor of Pharmacy (PharmD) degree from an accredited institution.Active, unrestricted pharmacist license.Ability to obtain additional state licensure as needed.Clinical KnowledgeStrong understanding of medication management principles and healthcare quality initiatives.Experience managing patients with chronic disease states and complex medication regimens.Knowledge of pharmacotherapy, medication safety, drug interactions, contraindications, and therapeutic monitoring.Ability to evaluate medication appropriateness using patient-specific clinical information.Communication & Patient EngagementStrong interpersonal and patient counseling skills.Experience engaging patients to improve medication adherence and health outcomes.Ability to communicate effectively with patients, providers, and interdisciplinary healthcare teams.Demonstrated professionalism, empathy, and emotional intelligence.Professional SkillsExcellent attention to detail and documentation practices.Ability to work independently while managing competing priorities.Strong organizational and time-management skills.Comfortable working in a performance-driven environment with measurable goals.Adaptable and collaborative approach to patient care and team interactions.Technical SkillsProficiency with common computer applications and web-based systems.Experience navigating multiple healthcare software platforms simultaneously.Ability to troubleshoot basic technical issues in a remote work environment.Preferred QualificationsExperience in ambulatory care, outpatient pharmacy, managed care, or population health settings.Background supporting chronic disease management programs.Experience working with older adult populations or long-term care patients.Familiarity with medication management and clinical intervention programs.Experience within health plans, PBMs, ACOs, or similar healthcare organizations.Prior work in high-volume patient outreach or clinical support environments.Experience with EMR/EHR platforms.Board certification (BCPS, BCACP, BCGP, or similar) preferred.Completion of a PGY1 and/or PGY2 residency preferred.Previous experience working remotely or in a telehealth setting.Work EnvironmentRemote position requiring a private, HIPAA-compliant workspace.Reliable high-speed internet connection required.Occasional travel may be required for training, team meetings, or company events.ScheduleFull-time schedule.Typical hours align with Pacific Time business operations.
