Employment Type: Full-TimeJob Summary:We are looking for a compassionate and experienced Registered Nurse (RN) to deliver high-quality, personalized nursing care to patients in a home care setting.

The ideal candidate will manage patients' health conditions, provide clinical assessments, and support recovery through skilled nursing interventions while promoting patient safety, comfort, and dignity in their own homes.

Key Responsibilities

Clinical Care & Monitoring:Conduct comprehensive patient assessments and develop individualized care plansAdminister medications, injections, IV therapy, and wound care as prescribedMonitor vital signs, symptoms, and medical equipment; document changes in patient conditionEducate patients and families on disease management, medication adherence, and home safetyPatient Support & Coordination:Coordinate with physicians, caregivers, and allied health professionals to ensure cohesive careMaintain accurate and timely nursing documentation and reportsRespond to emergencies and provide critical care intervention as neededEnsure compliance with infection control and hygiene standards in the home environmentCompliance & Quality Assurance:Follow care protocols as per DHA / MOH / DOH guidelinesConduct periodic reassessments and adjust care plans as necessaryParticipate in staff training, audits, and quality improvement initiativesQualifications & Requirements:Bachelor of Science in Nursing (BSN) or Diploma in NursingValid DHA / MOH / DOH license or eligibility is mandatoryMinimum 2 years of clinical nursing experience, preferably in home care or community healthCertified in Basic Life Support (BLS); Advanced Cardiac Life Support (ACLS) is a plusStrong communication and interpersonal skillsEmpathetic, detail-oriented, and capable of working independently
