Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents in Eureka and the surrounding Humboldt County area . What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Part-time position with a structured weekday schedule. You’ll spend 3 days per week delivering in-person care across Eureka and the surrounding area, with strategically clustered routes designed to minimize travel and maximize time with patients. Occasional telehealth visits add flexibility to your week. Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS Geriatrics or facility-based experience preferred; EMR proficiency Compensation Annual compensation range: $115,000–$130,000 (based on experience, schedule, and patient volume) Additional earning opportunities available based on coverage and panel needs W-2 with mileage reimbursed per policy Final terms outlined in the employment agreement On-site care with clustered routes to minimize travel 24/7 concierge support for scheduling, coordination, labs/orders, and pharmacy Modern tools that reduce admin and support same-day documentation and compliance Mission-driven care improving access and outcomes for seniors in assisted living
