This individual will independently manage medication order verification, clinical review, patient counseling, sterile compounding oversight, and pharmacy workflow decisions while serving as a key resource for technicians and interdisciplinary care teams.Position Summary:The Clinical Pharmacist owns the full clinical pharmacist scope for the pharmacy, including medication verification, therapeutic review, patient counseling, clinical interventions, and supervision of technician compounding and dispensing activities. This role serves as a safeguard for patient safety and regulatory compliance while supporting the delivery of high-quality specialty and infusion pharmacy services.Essential Functions:Clinical Pharmacy ServicesPerform prospective medication reviews, allergy screenings, and drug utilization reviews before dispensing medications.Verify medication orders and assess dose appropriateness utilizing patient-specific clinical information.Evaluate drug interactions, contraindications, therapeutic duplications, and opportunities for therapy optimization.Recommend dosage adjustments and alternative therapies when clinically appropriate.Assess complex patient situations and gather relevant clinical information from patients, providers, and healthcare teams.Counsel patients and caregivers regarding medication therapy, administration techniques, storage requirements, side effects, monitoring parameters, and treatment expectations.Document clinical interventions and patient care activities accurately and thoroughly.Dispensing & Compounding OversightConduct final verification and release of compounded sterile preparations prior to dispensing.Supervise pharmacy technician activities related to compounding, labeling, dispensing, and refill processing.Utilize established clinical references to address questions related to stability, sterility, beyond-use dating, labeling, and administration.Troubleshoot compounded products and determine appropriate corrective action when quality concerns arise.Regulatory & Compliance ResponsibilitiesMaintain compliance with all applicable Board of Pharmacy regulations, accreditation standards, USP guidelines, and company policies.Support inspection readiness activities and effectively explain workflows and regulatory processes during inspections and audits.Manage outdated medication and recall processes, including documentation, segregation, and disposal procedures.Ensure accurate documentation of verbal orders and pharmacy records.Operational ResponsibilitiesMaintain required pharmacist presence and ensure pharmacy security during assigned shifts.Participate in assigned after-hours on-call rotations and provide clinical support to patients and healthcare providers.Collaborate with nursing, intake, reimbursement, and provider offices to facilitate safe and timely patient care.Required QualificationsActive Pharmacist license in the state of practice.PharmD or Bachelor of Science in Pharmacy from an accredited institution.Two (2) to five (5) years of pharmacy experience, including infusion, specialty, hospital, ambulatory, or related clinical pharmacy practice.Demonstrated ability to independently perform: Clinical medication reviewOrder verificationPatient counselingClinical interventionsTechnician supervisionStrong knowledge of state and federal pharmacy regulations.Excellent communication, patient counseling, and interdisciplinary collaboration skills.

Preferred Qualifications
