This individual will independently manage medication order verification, clinical review, patient counseling, sterile compounding oversight, and pharmacy workflow decisions while serving as a key resource for technicians and interdisciplinary care teams.Position Summary:The Pharmacist in Charge owns clinical practice and regulatory compliance at a single Valley Vital Care site across its therapy lines. The PIC is accountable for medication safety, USP and execution, sterile-compounding oversight, clinical quality, the competency of site pharmacists and technicians, and faithful execution of the enterprise practice standard at the location. This is a hands-on site clinical leadership role for a pharmacist ready to own charge responsibility in a growth-stage, multi-site infusion environmentOwn the execution of the enterprise standard at the site. The PIC is measured on the levers that produce safe, compliant, consistent care at the location, medication safety, USP and and audit readiness, clinical quality, competency, and dyad health, while the COM retains ownership of site operations. The PIC is the single named accountable person for whether the site's pharmacy practice is safe, compliant, and to standard.ResponsibilitiesKey Responsibilities Clinical Practice & Regulatory ExecutionServe as the site Pharmacist in Charge of record; maintain compliance with state board of pharmacy, DEA, USP and , payer, and accreditation requirements at the site.Execute enterprise pharmacy policies, procedures, and clinical standards at the site; do not create parallel local standards outside the governed exception process.Oversee sterile compounding at the site: environment, garbing, technique, documentation, and testing per USP and .Maintain continuous audit and survey readiness; close site findings within agreed timelines and escalate systemic issues to the Director.Medication Safety & Clinical QualityOwn site medication-safety program execution: error and adverse-event capture, review, and corrective action, with systemic signals surfaced to the Director.Verify orders, compounds, and clinical appropriateness; serve as the site clinical resource to nurses, prescribers, referral sources, and patients on infusion therapy.Track site clinical-quality metrics and drive them to plan.Team Leadership & CompetencyLead, coordinate clinical coverage for, and develop site pharmacists and technicians; complete competency validation on the enterprise cadence.Onboard new pharmacists and technicians to the enterprise standard.Dyad & Cross-Functional PartnershipOperate the site dyad with the COM; align clinical practice with operational execution and escalate jointly when priorities intersect.Partner with Nursing, Intake, and Revenue Cycle at the site on throughput, clean clinical handoffs, and payer readiness before therapy starts.Note: Biologic Coordinators report directly to the Head of HR as part of the enterprise Access and operational strategy and sit outside the pharmacy clinical reporting line. The PIC partners with that function on therapy access and clinical standards but does not manage it.Responsibilities Owned by the Clinical Operations Manager (COM)The following operational responsibilities are owned by the COM and are not primary accountabilities of the PIC. The PIC provides clinical partnership and oversight as needed.Day-to-day site operations, staffing coordination, scheduling, and site-level labor management.Non-clinical teammate performance management and operational service recovery.End-to-end patient throughput and workflow integration across Intake, Nursing, Pharmacy, and Scheduling.Site operational SOP execution and monitoring.STRATEGIC DELIVERABLE: 30-DAY SITE PRACTICE SNAPSHOTWithin the first 30 days, the PIC delivers a written snapshot of the site's practice to the Director of Pharmacy. It is a required deliverable and a checkpoint for confirming role fit.It covers the compliance and safety state of the site, the maturity of standard adherence, competency status of the site team, the top three friction points, and a proposed stabilization plan for days 30 to 90.Pharmacist roles and responsibilities are also applicable to this positionQualificationsActive Pharmacist license in good standing required; PIC designation per state board where applicable; PharmD preferred.5 or more years of clinical pharmacy practice, including charge or PIC responsibility or demonstrated readiness to step into charge.Experience in infusion pharmacy, specialty pharmacy, acute care therapies, ambulatory infusion, or home infusion preferred.Strong command of USP and, sterile-compounding operations, and pharmacy regulatory requirements.Demonstrated team leadership, competency development, and the ability to hold direct accountability conversations; strong communication and organizational skills.Required SkillsStrong command of USP and , sterile-compounding operations, and pharmacy regulatory requirements.Demonstrated team leadership, competency development, and the ability to hold direct accountability conversations.Strong communication and organizational skills.Preferred SkillsExperience in infusion pharmacy, specialty pharmacy, acute care therapies, ambulatorySuccessful candidates will demonstrate:Sound clinical judgment and patient-focused decision-making.Commitment to medication safety and quality assurance.Strong attention to detail and regulatory compliance.Ability to effectively lead and support pharmacy technicians.Professional communication with patients, providers, and healthcare teams.Compensation & BenefitsAnnual base salary starting at $148,000.
