The physician will provide comprehensive primary care to adult patients, with an emphasis on preventive health, management of chronic conditions, continuity of care, and positive patient outcomes.The successful candidate will work closely with Avanta Health's clinical and support teams to deliver coordinated, efficient, and compassionate care. This position provides an opportunity to build lasting relationships with patients while contributing to the continued growth and development of our practice.Key ResponsibilitiesThe Primary Care Physician will be responsible for the following:Provide comprehensive primary care services to adult patients.Perform medical histories, physical examinations, assessments, diagnoses, and treatment planning.Provide preventive care, annual wellness visits, health screenings, and patient education.Evaluate and manage acute illnesses and chronic conditions, including diabetes, hypertension, COPD, cardiovascular disease, and other common medical conditions.Develop individualized treatment plans based on each patient's clinical needs, preferences, and goals.Prescribe medications and other appropriate therapies and monitor treatment effectiveness.Order, review, and appropriately follow up on laboratory tests, imaging, and other diagnostic studies.Maintain accurate, timely, and complete documentation within the electronic health record.Provide appropriate follow-up and promote continuity of care for an established patient panel.Coordinate care with specialists, behavioral health professionals, nurses, and other members of the healthcare team.Collaborate with clinical and administrative staff to support efficient clinic operations and patient flow.Participate in quality improvement, care management, and population health initiatives.Support appropriate clinical documentation and quality measures.Help patients navigate the healthcare system and facilitate appropriate referrals and follow-up care.Identify opportunities to improve clinical workflows, patient access, efficiency, and overall patient experience.Maintain professional standards of conduct, medical ethics, and patient confidentiality.Perform other related duties as assigned.Required QualificationsDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.Successful completion of an accredited residency program in Family Medicine, General Practice, or Internal Medicine.Board Certified or Board Eligible in Family Medicine or Internal Medicine.Current, unrestricted California physician license.Current DEA registration.Current BLS and ACLS certifications.Prior outpatient primary care experience preferred.Preferred Experience and SkillsCandidates who possess the following qualifications are encouraged to apply:Vietnamese-speaking ability is highly preferred.Experience providing care to older adults and geriatric patients is preferred.Background in value-based care, managed care, community health, or similar healthcare environments.Strong clinical assessment and decision-making skills.Excellent communication and interpersonal skills.Ability to collaborate effectively with physicians, nurses, medical assistants, and other healthcare professionals.Commitment to preventive care, chronic disease management, and patient education.Demonstrated commitment to providing compassionate and culturally responsive patient care.How to ApplyInterested candidates are encouraged to submit their resume for consideration.To apply: Please submit your current resume/CV through the application process.Schedule & Work EnvironmentThis is a full-time, onsite position located at:Avanta Health 11402 Warner Ave Fountain Valley CA 92708Schedule: Schedule varies based on physician availability and clinic needs.The anticipated salary range for this position is $200,000 to $400,000 per year. Compensation will be based on factors such as experience, qualifications, clinical background, schedule, and other job-related considerations.
