RTD Talent Solutions has been retained for a physician search by one of South Florida’s premier healthcare groups with more than 50 years of operations that is seeking a Primary Care Physician (MD). This is a permanent, full-time, on-site role based out of Tamarac or Miami Gardens, Florida.Job SummaryAs a Primary Care Physician, you will be responsible for delivering comprehensive outpatient medical care to Medicare Advantage patients, primarily those enrolled in Dual Eligible Special Needs Plans (DSNP).This role focuses on preventive medicine, chronic disease management, and individualized treatment planning to improve patient outcomes and overall quality of care. You will oversee patient assessments, coordinate specialty care, maintain accurate clinical documentation, and support regulatory compliance and quality initiatives, including HEDIS, CMS Star Ratings, and HCC risk adjustment.The ideal candidate will demonstrate a patient-centered approach while effectively managing complex medical conditions and collaborating with multidisciplinary healthcare teams.Key Job Duties:Patient Care & Treatment: Provide comprehensive primary care services, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.Preventive Care & Assessments: Conduct Annual Wellness Visits, health risk assessments, and preventive screenings to identify and address patient healthcare needs.Quality & Compliance: Support HEDIS measures, CMS Star Ratings, and HCC risk adjustment initiatives through accurate clinical documentation, preventive care, and chronic disease management.Care Coordination: Collaborate with specialists and interdisciplinary care teams to manage referrals, hospital discharge transitions, and continuity of care for Medicare Advantage and DSNP patients.Medication & Patient Management: Perform medication reconciliation, monitor treatment adherence, identify barriers to care, and coordinate resources to address patients’ medical and social needs.QualificationsMedical Doctor (MD) with an active Florida medical license or valid Florida Area of Critical Need (ACN) certificate authorizing practice at the assigned location.Board certification in a primary care specialty.Experience treating Medicare Advantage and DSNP populations, older adults, and patients with multiple chronic conditions.Spanish proficiency required; English proficiency preferred.Strong understanding of preventive care, chronic disease management, and healthcare quality measures.
