It is the only primary care provider to carry the AARP name, reflecting its commitment to the needs of older adults.Role Description The Primary Care Provider – Value Based Care is a full-time, on-site role based in Newark, NJ. This role focuses on delivering comprehensive, patient-centered primary care to adults on Medicare using a value-based care model. Day-to-day responsibilities include performing medical assessments and examinations, diagnosing and managing acute and chronic conditions, prescribing medications, and coordinating care plans that emphasize prevention and long-term health outcomes. The provider collaborates closely with an interdisciplinary care team, participates in case reviews, and leverages data and quality metrics to improve patient outcomes and practice performance. Additional responsibilities include patient education, documentation in electronic medical records, and participation in quality improvement and population health initiatives.QualificationsStrong Communication skills, including clear patient education, team collaboration, and documentation.Clinical Medicine expertise in adult and geriatric primary care, including diagnosis, treatment, and preventive care.Customer Service skills with a focus on empathy, respect, and building long-term patient relationships.Training skills to support patient self-management and to mentor or guide care team members.Experience or familiarity with Home Care coordination, including safe transitions, community resources, and follow-up.Active and unrestricted medical license (MD, DO, or NP) in New Jersey.Board certification or eligibility in Internal Medicine, Family Medicine, or relevant primary care specialty.Experience in value-based care, population health, or working with Medicare populations is highly beneficial.Comfort using electronic medical records (EPIC) and data to guide clinical decisions and quality improvement.Commitment to health equity and providing care in historically underserved communities.
