Registered Dietitian

LaSante Health Center is seeking a knowledgeable and compassionate Registered Dietitian (RD/RDN) to join our growing healthcare team. This full-time, on-site position is based in Brooklyn, NY.The Registered Dietitian Nutritionist will provide individualized nutritional counseling and education to patients, with a focus on weight management, chronic disease prevention and management, and overall wellness. The ideal candidate will be comfortable working with a diverse patient population and collaborating with medical providers and other healthcare professionals to support comprehensive patient care.ResponsibilitiesConduct nutritional assessments and evaluate patients' individual dietary needs and health goals.Provide one-on-one nutritional counseling and education to patients.Develop personalized meal plans and nutrition recommendations based on patients' medical conditions, lifestyles, and goals.Support patients with weight management, obesity, diabetes, hypertension, cardiac health, and other nutrition-related health concerns.Educate patients on healthy food choices, portion control, meal planning, and sustainable lifestyle changes.Collaborate with physicians, nurses, and other healthcare providers to develop comprehensive patient care plans.Monitor patient progress and adjust nutrition plans and recommendations as needed.Maintain accurate and timely patient documentation.Provide nutrition education that is culturally sensitive and appropriate for a diverse patient population.Stay current on evidence-based nutrition practices and relevant healthcare guidelines.QualificationsCurrent Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential is required.Experience in dietetics, nutritional counseling, and nutrition education.Experience with weight management and creating individualized meal plans.Knowledge of nutrition-related chronic conditions, including diabetes, obesity, hypertension, and cardiovascular disease.Strong interpersonal and communication skills.Ability to effectively engage and educate a diverse patient population.Ability to work collaboratively as part of a multidisciplinary healthcare team.Knowledge of community-specific health challenges is a plus.About LaSante Health CenterLaSante Health Center is a culturally diverse medical facility providing comprehensive medical, dental, behavioral health, and specialty care to patients of all ages.
