🚨 Now Hiring: RN Care Manager 🚨📍 Location: White Plains, NY 10601💰 Salary: $126,160.32 – $160,090.32🕘 Schedule: Full-Time | Monday–Friday, 9:00 AM–5:00 PM | Weekend rotation (1 Saturday & Sunday per month)We're seeking an experienced RN Care Manager to join a collaborative acute care team focused on care coordination, utilization review, and safe discharge planning.QualificationsRequired✔️ BSN required (or MSN)✔️ Active NYS Registered Nurse (RN) license✔️ MUST have hospital discharge planning and/or acute care management experience✔️ 2+ years of experience in an acute care hospital✔️ Strong computer skillsPreferred✔️ Previous hospital case management experience✔️ PRI Certification (or ability to obtain within 3 months of hire)✔️ SCREEN Certification✔️ Care Management Certification (CCM, CMC, or equivalent)✔️ 3+ years of Emergency Department, Critical Care, or Medical-Surgical nursing experience✔️ Knowledge of healthcare financial, regulatory, and payer requirements✔️ Knowledge of state, local, and federal healthcare programs✔️ Experience using Milliman and/or InterQual criteriaWhat You'll DoComplete comprehensive patient assessments and identify discharge planning needs from admission through discharge.Coordinate multidisciplinary care and collaborate with physicians, nurses, social workers, and community providers.Develop and implement safe discharge plans to reduce readmissions and improve patient outcomes.Perform utilization review and ensure compliance with payer and regulatory requirements.Educate patients and caregivers while coordinating post-acute services and community resources.If you're an experienced RN with a strong background in hospital discharge planning and care management, we'd love to hear from you!📩 Apply today or message me directly for more information.Rebecca Coursey- Rcoursey@tagmedstaffing.com
