As a diabetes educator, this RN will assess patient needs, develop individualized care plans and participate in interdisciplinary care meetings. As a case manager this RN works to improve access and effectiveness of care delivery system to improve patient outcomes.Ability to travel within the State of Nevada.Essential Duties and ResponsibilitiesDirect patient education in-person and virtually.Provide medication and insulin teaching to patients.Provide glucose monitoring support to patients.Provide lifestyle counseling to promote optimal self-management with patients with Diabetes and other chronic diseases.Provide and participate in care coordination with primary care and specialistsCoordinate and conduct home visits as needed.Interpret data and trends from multiple data sources including remote patient monitoring, using appropriate analytical and critical thinking skills.Provide timely and accurate documentation, tracking and reporting of diabetic clinical outcomes.Provides timely responses to inquiries from health plans, providers and other members of the patient care team.This RN will collaborate with medical, dental and operational leadership to develop a comprehensive Medical assistant training curriculum.Other duties as assigned by directed.Required QualificationsGraduate of an accredited nursing schoolLicensure as a registered nurse in the state of NevadaCurrent CPR or BLS certificationActive Nevada Driver's LicenseA minimum of three years of experience in a clinical practice with good clinical skills/ability to function as a staff nurse is requiredPreferred QualificationsKnowledge of the essential functions, practices and procedures of a medical clinic and officeKnowledge and skill in pediatric and adult medication and immunization administration and storage and handling practices.Excellent ability to problem solve, de-escalate/resolve conflict and perform service recovery.Equal Opportunity StatementNevada Health Centers will provide equal opportunity employment to all employees and applicants for employment.
