As Deaconess grows, it seeks skilled, compassionate professionals who want to make a positive impact on patients, families, colleagues, and the community.Role Description The RN Wellness Coach is a full-time, on-site role based in Evansville, IN, focused on supporting patients in achieving and maintaining optimal health and wellness. In this role, the nurse will provide individualized coaching, education, and care coordination to help patients manage chronic conditions, adopt healthy lifestyle behaviors, and navigate available medical and community resources. Daily responsibilities may include conducting wellness assessments, developing personalized care plans, monitoring progress, and collaborating with physicians, interdisciplinary teams, and community partners to ensure continuity of care. The RN Wellness Coach will document patient interactions in accordance with regulatory and organizational standards, track outcomes, and participate in quality improvement initiatives. The role also involves delivering group education sessions, promoting preventive care, and encouraging patient engagement through clear communication and motivational support.Qualifications Active Registered Nurse (RN) license in the state of Indiana (or eligibility for licensure) and graduation from an accredited nursing program.Experience in wellness coaching, care management, or population health, with skills in patient education, health promotion, and chronic disease management.Strong clinical assessment and critical thinking abilities, including monitoring patient status, identifying risks, and developing appropriate care plans.Excellent communication and interpersonal skills, with the ability to build trust, motivate behavior change, and work effectively with diverse patient populations.Ability to collaborate within interdisciplinary teams, coordinate care across settings, and use sound judgment in fast-paced clinical environments.Proficiency with electronic health records and basic computer applications, along with accurate documentation and organizational skills.Commitment to patient-centered care, cultural sensitivity, and maintaining confidentiality and professional ethics.Prior experience in ambulatory care, community health, or wellness programs is preferred; certification in health coaching or related specialty is a plus.
